Finally! Kicking off Spay Day 2008, a day of cat snipping, islandwide!
This is yet another effort by CWS to thank the caregivers by offering free sterilisation slots, in conjunction with participating vets!
>>> Please note that we are not able offer a vet of your choice. The slots will be allocated according to the areas in which you would deliver your cat(s) to - North/South/East/West. If you are unsure of your area, please do include your mailing address and we will arrange this accordingly.
To register the quickest way:
Download the consent form, and here are the Chinese version and the Malay version of the form...
Fill it up, sign it and mail it back to us at:

Please leave your contact details and mailing address and we will mail the consent form out for you to fill out. Please sign it and mail it back to us - before we confirm your registration.
NOTE: Please do not send the soft-copy of the application form via email - we need the printed and signed copy to confirm your booking. So it is best if you could print out the forms via the links above and mail it back to us.
